The Crucial Distinction: Medical Necessity vs. Cosmetic Enhancement

The foundational rule set by the IRS for using is simple: the procedure must be primarily for the diagnosis, cure, mitigation, treatment, or prevention of disease. If the primary purpose is purely to improve appearance, it is generally not covered.

This is why treatments at a medically overseen facility like Front Door MedSpa—where a licensed medical provider and a cardiologist are involved in patient care—can sometimes qualify with the proper documentation.

Treatments That May Qualify With a Letter of Medical Necessity

While purely hsa eligible cosmetic procedures are typically restricted, certain treatments become eligible when they are prescribed by a physician to treat a diagnosed medical condition. These often require a Letter of Medical Necessity (LMN) from your primary care doctor, outlining the diagnosis and why the treatment is essential.

1. Chronic Pain and Tension Relief

  • Botox for Chronic Pain: Cosmetic injections to smooth wrinkles are generally not eligible. However, if you suffer from chronic pain treatments fsa hsa such as chronic migraines, TMJ (jaw tension), or muscle spasticity, use fsa for botox injections may qualify. The treatment must be prescribed specifically to alleviate the medical condition.
  • Acupuncture or Therapeutic Treatments: While we do not offer traditional massage therapy or accupuncture treatments, services aimed at pain mitigation may be eligible if a physician prescribes it to treat a specific ailment.

2. Managing Skin Conditions and Disease

  • Acne Treatments: Acne is a diagnosed skin condition, making targeted treatments eligible. Services used for acne treatment can include chemical peels, specialized facials, or laser treatments, provided they are explicitly directed at managing the acne. Over-the-counter acne medicines are often eligible as well.
  • Rosacea and Psoriasis: Treatments directed at diagnosing or alleviating the symptoms of skin diseases like rosacea or psoriasis are strong candidates for eligibility. Any service used to improve skin health by addressing a medical diagnosis may qualify with an LMN.

3. Physician-Directed Weight Loss Programs

  • Weight Loss: Cosmetic fat reduction (like CoolSculpting® Elite) is not eligible. However, weight loss programs are eligible if they are a prescribed treatment for a disease diagnosed by a physician, such as obesity, hypertension, or heart disease.

4. Varicose or Spider Vein Treatments

  • Varicose Vein Treatment / Spider Vein Treatment: If these procedures are done to alleviate pain, aching, or functional symptoms (like swelling or inflammation) rather than just improving appearance, they may qualify. Our Medical Director, an Interventional Cardiologist, provides essential oversight for these vascular treatments.

Medspa Services Generally Not Covered

It is critical to remember that if a treatment’s primary purpose is cosmetic, it will not be covered by your FSA or HSA. This includes, but is not limited to:

  • happy smiling manWrinkle Reduction: Purely aesthetic botox to smooth forehead wrinkles or crow’s feet is not covered.
  • Aesthetic Enhancement: Dermal fillers for lip plumping, cheek contouring, or achieving a fsa eligible skin tightening look purely for aesthetic reasons. Although usually cosmetic, dermal fillers may be eligible if used for reconstructive purposes following trauma, illness, or to correct a disfiguring deformity
  • Laser Hair Removal: Generally considered cosmetic, laser hair removal is not eligible for reimbursement because it does not treat or prevent disease.
